WAHA Annotation of Birth Facts Form
PROOF OF AGE FORM
Wisconsin Amatuer Hockey Association (WAHA) requires that players provide proof of age documentation. Please present the form to the right along with a legal certified copy of the player's birth certificate at the parent meeting prior to the season. When the form and certificate are presented, two board members will validate the information and sign the form. The form will be put on file with DPYH.
Forms only need to be submitted one time for each player for the duration of the time they play with DPYH.
